Understanding Auditory Brainstem Implants: A Breakthrough in Hearing Loss Diagnosis

The cochlear nucleus is the first stop in the brain's processing of sound, marking the beginning of the auditory pathway. From here, low-frequency sounds from the apical portion of the cochlea and high-frequency sounds from the basal portion travel separately to the central and peripheral parts of the auditory nerve (1). The unique tonotopic organization of the cochlea ensures that high-frequency sounds stimulate the basal component while low-frequency sounds stimulate the apical component.

The cochlear hair cells convert sound waves into electrical signals, which are then transmitted to the spiral ganglion and eventually reach the brainstem through the auditory nerve or vestibulocochlear nerve (cranial nerve VIII) (2). Auditory Brainstem Implants (ABRs), also known as Brainstem Auditory Evoked Potentials (BAEPs), have revolutionized the diagnosis of hearing loss, acoustic tumors, and cerebellopontine angle (CPA) tumors.

Introduced in audiology in the 1970s, ABRs have become increasingly important with the advent of contemporary guidelines for newborn hearing screening. This non-invasive test measures synchronous neural fiber activity along the auditory pathway to determine hearing thresholds (3). The readings consist of a sequence of up to seven positive wave peaks labeled I-VII, accompanied by negative troughs in between.

During an ABR test, electrical activity from the eighth cranial nerve and neurons along the brainstem auditory pathway is recorded using surface electrodes placed on the scalp, forehead, and near the ears for 10 milliseconds after an acoustic stimulus (4). This objective measurement of auditory pathway function from the auditory nerve to the mesencephalon provides valuable insights into hearing sensitivity thresholds in individuals who are unable to tolerate traditional behavioral audiometry.
